New Hampshire Success Story – Physical Activity Vital Sign Embedded in Centricity Electronic Health Record: Creating digital workflows to increase patient physical activity
Under cooperative agreement, DP18-1803, State Public Health Approaches to Addressing Arthritis, the New Hampshire Department of Health and Human Services partnered with federally qualified health centers and the Granite YMCA to integrate physical activity screening and referral to arthritis-appropriate, evidence-based interventions (AAEBIs) into the existing electronic health record (EHR) system. During the 18-month pilot program 6,824 patients were screened using the Physical Activity Vital Sign EHR module. Of those, 456 participants were enrolled by the YMCA in one of four different physical activity programs. Findings showed that when integrated into existing workflows within an EHR, physical activity screening, counseling, and referral to community-based lifestyle management programs are feasible without increasing provider charting time or decreasing productivity levels. |
